Application for Provisional Certification

State certification ensures that high quality health education services are offered to Arkansas citizens by qualified health educators.  State certification:

  1. attests to the professional health educator’s knowledge and skills essential to the practice of health education

  2. assists employers in identifying qualified practitioners

  3. assures consumers of the validity of services offered.

To become State Certified, you must:

  1. Complete an application to the Arkansas Board of Health Education

  2. Provide evidence of certification as a Certified Health Education Specialist by the National Commission for Health Education Credentialing

  3. Pay a one-time fee of $25.00.

Mail the Application for Provisional Certification form along with a check for the $25 registration fee to: Arkansas Board of Health Education, P.O. Box 251342, Little Rock, AR  72225.
